  • Pre-printed History, Progress, Insurance tabs speed setup
  • Erasable blank tab adapts to any record type

Ready-to-use chart divider sets

Each box contains one hundred sets of four pre-collated dividers made from 90lb manila stock. The sheets measure 11 inches tall and include side tabs that extend half an inch beyond the page edge. Supplied in sequence, they are ready for immediate filing without assembly.

Side-tab layout for quick reference

Side-positioned tabs allow rapid visual scanning along the binder edge. Four distinct sections per set separate History & Physical, Progress Notes, Insurance and a blank write-on erasable tab. The 1/4th cut stagger keeps labels visible when charts are stacked or stored upright.

Suited to primary care practices

Designed for clinics that file patient records in standard letter-size binders and need consistent, colour-coded sections. Not suited for offices that require legal-size dividers, top-tab configurations or a different tab sequence than the four supplied.

The 90 lb manila stock is a heavyweight paper grade designed to withstand frequent handling in busy primary care practices without tearing or curling.

Will the side tabs and manila stock create bulk or heat in a packed cabinet?

These are passive paper dividers with side tabs extending half an inch; they generate no heat, make no noise, and draw no power.

Do the pre-printed History & Physical, Progress Notes, Insurance, and blank erasable tabs match my existing chart layout?

Each set includes those four specific tabs in blue, yellow, brown, and a blank write-on erasable tab; verify your chart order before ordering.
